Laparoskopik sleeve gastrektomi (LSG) yaşam şekli değişikliğine ve farmakolojik tedaviye rağmen kilo veremeyen obez hastalarda bir tedavi seçeneğidir. Yağ dokudan salınan hormonlar olan adipokinlerin besin alımı, enerji dengesi, glukoz ve lipid metabolizmasındaki etkileri bilinmektedir. Endoplazmik retikulum (ER) stresi metabolik hastalıklar, kanser, immun hastalıklarla ilişkilendirilmiştir. Bu çalışmada LSG operasyonu yapılan obez hastalarda serum Wingless-type inducible signaling protein-1 (WISP1), Neuregulin 4 (Nrg4), Asprosin, Speksin (SPX) adipokinleri, serum ER stresi belirteçleri Glikozla düzenlenen protein 78 (GRP78), X-box bağlayıcı protein 1 (XBP1), Aktive edici transkripsiyon faktörü 6 (ATF6), CCAAT / güçlendirici bağlayıcı protein (C / EBP) homolog protein (CHOP) düzeylerinin zamana bağlı değişimleri ile birlikte obez hastalarda yağ doku WISP1, Nrg4, Asprosin, SPX düzeylerinin araştırılması amaçlanmıştır. Tez kapsamında 18-64 yaşlarında LSG operasyonuna alınan, ek hastalığı olmayan morbid obez hastalar (n=19) ve kontrol grubu olarak normal kilolu olup kolesistektomi veya abdominal herni ameliyatı olan bireyler (n=19) karşılaştırılmıştır. LSG operasyonu olan hastalardan preop, 1. ay, 3. ay ve 6. aylarda serum örnekleri, kontrol grubundan ise preop serum örnekleri alınmıştır. Her iki gruptan ameliyat esnasında omentum ve subkutan yağ doku çıkarılmıştır. Adipokinlerin ve ER stresi belirteçlerinin düzeyleri ticari kitler ile ölçülmüştür. İstatistiksel analiz için Mann Whitney-U, t testi, Friedman testi, varyans analizi kullanılmıştır. LSG operasyonu sonrası obez hastaların vücut kitle indeksleri (VKİ), toplam yağ, gövde yağ miktarları ve yağ yüzdeleri azalmıştır. Obez hastaların postop serum glikoz, insülin, HOMA-IR ve trigliserid düzeyleri düşmüş, HDL düzeyleri artmıştır. Obez hastalarda kontrol grubuna göre farklı olmayan preop LDL ve total kolesterol düzeyleri postop 6. ayda kontrol grubuna ve 1. aya göre yüksek bulunmuştur. Obez hastalarda omentum WISP1 düzeyi, subkutan yağ doku WISP1 ve SPX düzeyleri ile serum WISP1, Nrg4, Asprosin ve CHOP seviyelerinin sağlıklı bireylerden yüksek olduğu gözlenmiştir. Serum ATF6 ve CHOP düzeyleri postop 3. ayda azalmış ancak 6. ayda preop değere göre anlamlı bir fark gözlenmemiştir. Verilerimiz diyet, egzersiz ve farmakoterapiyle kilo veremeyen obez bireylerde LSG operasyonunun genel olarak faydalı etkileri olduğunu göstermektedir.
Laparoscopic sleeve gastrectomy (LSG) is a treatment option for obese patients who cannot lose weight despite lifestyle changes and pharmacological treatment. The effects of adipokines on food intake, energy balance, glucose and lipid metabolism are known. Endoplasmic reticulum (ER) stress has been associated with cancer, metabolic and immune diseases. In this study, we aimed to investigate the time-dependent alterations of serum Wingless-type inducible signaling protein-1 (WISP1), Neuregulin 4 (Nrg4), Asprosin, Spexin (SPX) adipokines, serum ER stress marker levels such as Glucose regulated protein 78 (GRP78), X-box binding protein 1 (XBP1), Activating transcription factor 6 (ATF6), CCAAT / enhancer binding protein (C / EBP) homologous protein (CHOP), as well as adipose tissue WISP1, Nrg4, Asprosin, SPX in obese patients who underwent LSG surgery. Morbidly obese patients (n=19) without any additional disease who underwent LSG operation between the ages of 18-64 and individuals with normal weight who had cholecystectomy or abdominal hernia surgery (n=19) were compared. Serum samples were obtained from the patients who had LSG operation at preoperative, 1st, 3rd and 6th months, and preoperative serum samples from the control group. Omentum and subcutaneous adipose tissue were removed from both groups during the operation. Levels of adipokines and ER stress markers were measured with commercial kits. Mann Whitney-U, t test, Friedman test, analysis of variance were used for statistics. Body mass index (BMI), total fat, body fat amount and fat percentage of obese patients decreased after LSG. Postoperative serum glucose, insulin, HOMA-IR and triglyceride levels of obese patients decreased and HDL levels increased. In obese patients, preoperative LDL and total cholesterol levels, which were not different from the control group, were found to be higher in the postoperative 6th month compared to control group and 1st month measurements. Omentum WISP1 levels, subcutaneous adipose tissue WISP1 and SPX levels, and serum WISP1, Nrg4, Asprosin and CHOP levels were observed to be higher in obese patients. Serum ATF6 and CHOP levels decreased in the postoperative 3rd month, but no significant difference was observed in the 6th month compared to the preop value. Our data show that LSG surgery has generally beneficial effects in obese individuals who cannot lose weight with diet, exercise and pharmacotherapy.
